Survivors can file losses for the 2011-2012 tax year or the current year (2012-2013) using Form 4684 - Casualties and Thefts.

To file losses for the 2011-2012 tax year, one would have to use Form 1040X – Amended Tax Return.

Those filing may find Publication 2194 - Disaster Resource Guide for Businesses and Individuals to be helpful.

Tax Forms

To assist people with filing federal and state income tax, the Monmouth County Library maintains a collection of the most common federal and some New York tax forms during the tax season. In addition, the Library maintains a set of reproducible federal, New Jersey (prior years), and New York tax forms which may be copied. Tax forms and publications may also be downloaded from the Internal Revenue ServiceState of New Jersey Division of Taxation and New York State Department of Taxation and Finance.

We were notified by the State of New Jersey Division of Taxation that for the year 2009 the Division will not issue income tax forms to any of their distribution points. This is a direct result of the current economic situation. 


Tax Assistance

Between February 1st and April 15th, the AARP offers its Tax-Aide counseling program at some of the Monmouth County Library Branches. Trained and certified AARP Tax-Aide volunteer counselors help low- to middle-income taxpayers prepare and file their income tax returns with the IRS , free of charge. Check our February, March, and April Adult Calendar of Events (when available) to view participating branches and hours.
